Your Vet will supply us with a copy of your pet's relevant medical history and test results (including blood results, X-rays etc.) either via e-mail, fax or post in advance of your appointment. As a backup, they may ask you to bring the records with you (particularly if your referral is urgent) - it is always best to double-check with your vet. Please also bring any current medications / prescription foods with you.
It can be helpful to make some brief notes regarding your pet's problems eg. the dates upon which problems have developed, when certain treatments have been given and what effect, if any, you observed. Some of these areas are covered in our general health questionnaire; it is most helpful to your pet if all of his/her carers contribute to this questionnaire prior to your appointment, and that the principal carer(s) attend the appointment. It is also worth making a check-list of your own queries, so that by the end of the consultation you can make sure that we have addressed any outstanding concerns.
Please bring with you any relevant insurance documentation, including a blank Veterinary Fees Claim Form from your pet insurance provider. You will also need to bring a method of payment.
Please remember to pack any medicines/supplements, special foodstuffs, special harnesses or particular blankets/bedding/toys that you would like your pet to have whilst in hospital.
